  • Donation-related information processed by Stripe. CBOMH does not store complete payment-card numbers on this website.

Please do not submit health information

CBOMH is a community resource and connection organization, not a healthcare provider, health plan, or emergency service. Our public forms and routine communications are not intended for diagnoses, treatment records, insurance records, Social Security numbers, or other sensitive medical information. Please do not submit protected health information or detailed clinical information.

How we use information

We use the information you provide to respond to requests, connect people with relevant public or community resources, review partnership applications, administer donations, improve the website, prevent misuse, and meet legal or accounting obligations.

Service providers

We may use trusted service providers to operate forms, communications, hosting, analytics, and payments. These currently include GoHighLevel/LeadConnector for forms and workflow communications, Stripe for donation processing, and our website-hosting providers.
